NOTE: Fees are payable by cash or money orders only (made payable to the Embassy of the Czech Republic). Checks or credit cards cannot be accepted! The fees may change without prior notice.

Citizens of the United States traveling to Czech Republic must have a valid passport (that does not expire before the end of their visit), but do not require a visa for stays of less than 90 days within a six-month period.

Other travelers may apply for a Schengen visa, which allows them to enter Czech Republic or another Schengen country and then to travel freely throughout the Schengen zone for a period not to exceed 90 days.

Theft or loss of a valid passport must be reported to the police and a police report should be presented when applying for a new passport. Otherwise a passport is applied as normally. When reporting a lost passport to the police it should be noted that a report made by telephone and a registration number given  on the basis of that might not be adequate proof to the insurance company. The report can be made in person at a police station so that a report of the crime is available in writing
