The Embassy offers a wide range of consular services:

  • Vital Statistics Documents (Birth/Marriage/Death certificates), etc.

The passport applicant residing in the consular territory of the Czech Embassy in Ottawa contacts the Embassy via e-mail and attaches The Request for Verification of Personal Information in the Civil Register form. Once the Embassy confirms via e-mail the correctness of the entry in the Civil Register, the applicant visits the Embassy in person (previous appointment is recommended) and submits all the required documents. The consular officer will photograph the applicant, scan their fingerprints and forward the application in electronic form for further processing to the central passport authority in the Czech Republic. The passport will then be manufactured and forwarded to the Embassy. The applicant will be notified and will have to, within the period of 6 months, personally collect the document at the Embassy.

The legal processing time is 120 days when applied for abroad. The average waiting times are 40-60 days abroad, 10-20 days in the Czech Republic.

All vital events (i.e. birth, marriage, death) of Czech citizens are registered by the office of "Zvláštní matrika" in the city of Brno, Czech Republic. Applications for birth certificates, marriage certificates and death certificates can be submitted to this office directly or through the Embassy.

The Consulate only accepts cash, money orders or certified cheques (made payable to the Consulate General of the Czech Republic). If one intends to mail the application and wishes to pay by cheque or a money order, it is suggested to send a few dollars more to ensure there is a sufficient amount of money to cover all the application fees.

The Embassy is closed on the following days:
