- What You'll Do

In this Home Health LPN role, you will:

* Provide skilled nursing care in the home under the direction of an RN and in accordance with the physician's Plan of Care

* Review and follow the Plan of Care prior to each visit to ensure safe, effective treatment

* Document patient assessments, care provided, and progress accurately and timely

* Communicate changes in patient condition, safety concerns, or environmental risks to the RN Case Manager and care team

* Assist patients and caregivers with education on medications, treatments, and self-care techniques

* Support patient safety using family and community resources

* Collaborate closely with RN Case Managers during normal business hours for guidance and clinical support

* Maintain accurate medication documentation and update medication profiles per agency guidelines

* Deliver compassionate, respectful care to patients, families, and caregivers

* Take proactive steps to help prevent avoidable hospitalizations and emergency room visits

- What You'll Need

* Graduate of an accredited Practical Nursing program (Certificate, Diploma, or Associate Degree)

* Active state license as a Licensed Practical Nurse (LPN)

* Minimum of 1 year of clinical nursing experience

* Home health experience preferred

* Ability to travel up to 50% within the assigned territory

* Ability to sit, stand, bend, lift, and move intermittently; must be able to lift up to 50-100 lbs

* Valid driver's license, current auto insurance, and reliable transportation
