Allure of Zion – Supportive Living Facility (SLF) is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide direct nursing care and oversee day-to-day nursing activities. This role includes supervising nursing assistants and ensuring care is delivered in accordance with federal, state, and local regulations.

• Provide direct nursing care to residents per physician orders and care plans

• Supervise and direct day-to-day activities of nursing assistants

• Admit, transfer, and discharge residents as required

• Complete required documentation related to admissions, transfers, and discharges

• Receive and transcribe physician telephone orders

• Prepare and administer medications as ordered

• Complete routine nursing documentation and nurse’s notes

• Investigate, document, and report accidents/incidents to the Director

• Participate in shift change reports and interdisciplinary communication

• Promote a calm, respectful, and resident-centered care environment

• Maintain confidentiality of resident information and PHI

• Perform other duties as assigned

• Current, unencumbered LPN license (Illinois)

• CPR certification required

• Ability to read, write, speak, and understand English

• Ability to make independent nursing decisions when appropriate

• Knowledge of nursing practices, medical procedures, and regulations related to Supportive Living Facilities

• Prior experience preferred

• Ability to move intermittently throughout the workday

• Ability to meet facility health requirements

• Ability to lift, push, pull, or carry up to 25 pounds and transport items up to 50 feet

• Ability to assist with emergency evacuations if necessary
