Key Responsibilities:
- Conduct comprehensive assessments and wellness exams in-home and virtually.
- Educate patients and families on health benefits and programs.
- Coordinate care with healthcare providers and maintain accurate patient records.
- Deliver evidence-based care to reduce hospitalizations and enhance patient satisfaction.
Requirements:
- Bilingual in English and Spanish.
- Active RN and Nurse Practitioner or Physician Assistant license.
- 2+ years of direct patient care experience.
