Mid = $62.81

The Transfer Coordinator assists with processing patient admissions and transfers. The Coordinator is responsible for facilitating transfers from outside facilities and managing patient flow and bed placement. This individual occupying this position must demonstrate appropriate, diplomatic, and sensitive communication and interaction skills, all while working in a fast‑paced, acute healthcare environment with multiple distractions and ever‑changing priorities.
Responsibilities- Provide direction, guidance, and coordination to healthcare providers for optimal, safe, effective and timely patient placements and movement and manage in‑coming acute care transfers, inpatient outgoing acute care transfers and direct admissions.
- Communicate with referring hospital to coordinate transportation arrangements, provide assigned beds and assist with screening and facilitating unscheduled/urgent admissions.
- Connect sending and accepting physicians by a physician‑to‑physician call.
- Interact with physicians, nursing staff and EVS to effectively communicate patient bed placements to coordinate patient throughput. Assist case management with outgoing acute transfers from the emergency department as needed.
- Ensure safe and timely placement of patients based on admission status, physician‑ordered level of care, patient’s clinical information and nursing units’ admission and discharge criteria.
- Maintain documentation of activities in appropriate logs and track and trend barriers to patient intake flow.
- Exhibit and model exceptional customer relationship behaviors, including telephone and personal contact.

Associates Degree Licensure required:
Registered Nurse with Arizona BLS (AHA). Three+ years of RN experience. Case Management experience preferred.
