The Care Coordinator is responsible for overseeing a caseload of HCBS members receiving in-home support, which entails coordinating member (client) care, scheduling shifts, creating care plans, and orienting new staff to UCPSA's members (clients). The position requires standby availability, some weekends, evening work, and a flexible schedule. (Being on standby is scheduled with plenty of time, and it is on a rotation basis among a team of approximately 12 Care Coordinators and Staff Supervisors.)
The Care Coordinator position is critical in ensuring effective and efficient delivery of care services. The candidate should possess excellent communication and interpersonal skills, be able to work independently, handle multiple tasks, and make decisions under pressure. They must prioritize tasks, problem-solve, and provide excellent customer service. The required qualifications should be met.
