Position: Care Coordinator (Case Manager)

The Care Coordinator is responsible for overseeing a caseload of HCBS members receiving in-home support, which entails coordinating member (client) care, scheduling shifts, creating care plans, and orienting new staff to UCPSA's members (clients). The position requires standby availability, some weekends, evening work, and a flexible schedule. (Being on standby is scheduled with plenty of time, and it is on a rotation basis among a team of approximately 12 Care Coordinators and Staff Supervisors.)

The Care Coordinator position is critical in ensuring effective and efficient delivery of care services. The candidate should possess excellent communication and interpersonal skills, be able to work independently, handle multiple tasks, and make decisions under pressure. They must prioritize tasks, problem-solve, and provide excellent customer service. The required qualifications should be met.

QUALIFICATIONS
  • 18 years of age or older
  • Ability to obtain a current CPR/First Aid certification
  • Ability to obtain fingerprint clearance/DES background check
  • Ability to obtain a current T.B. clearance card
  • The ability to work in a team environment
  • The ability to work a flexible schedule
  • The ability to learn and operate UCPSA program software
  • The ability to lift and transfer clients
  • Own/have regular access to be able to operate a motor vehicle for the purpose of visiting and transporting clients, attending meetings outside of the office, etc. Proof of current registration and insurance of the vehicle must be kept on file with UCPSA
  • A degree in a related field, case management experience required, and at least one year of direct care experience, personally or professionally,
  • Bilingual in English and Spanish required
