We’re seeking an experienced Occupational Therapist (OT) for a contract assignment in a Skilled Nursing Facility (SNF) located in Yuba City, California. This role offers guaranteed hours and the opportunity to make a meaningful impact on patient recovery and rehabilitation.
Location: Yuba City, CA
Setting: Skilled Nursing Facility (SNF)
Productivity: 90% required
Orientation: 4 hours (unbillable)
RequirementsActive Occupational Therapist License (California)
BLS certification (required)
Minimum 1 year of recent SNF or rehab experience preferred
Must meet pre-employment credentialing requirements (background check, drug screen, etc.)
ResponsibilitiesEvaluate, plan, and implement individualized treatment programs for patients
Assist patients in improving functional independence in daily living activities
Collaborate with physicians, nurses, and rehab staff to coordinate patient care
Document patient progress and adjust treatment plans as necessary
Provide education to patients and families on rehabilitation techniques and home exercise programs
Ensure compliance with facility policies, procedures, and state regulations
