• Evaluate and treat residents to improve mobility, safety, and independence
• Create individualized care plans aligned with resident goals
• Collaborate with interdisciplinary team members
• Educate residents and families on progress and care plans
• Complete accurate and timely documentation

• ✔ Licensed Physical Therapist (PT) in Michigan (or eligible)
• ✔ Experience in or interest in senior living or skilled nursing
• ✔ Positive, adaptable, and team-oriented mindset
• ✔ Passion for high-quality, patient-centered care
