• Enable patients to spend quality time with their loved ones and pursue activities they enjoy
• Assess patients for hospice eligibility and explain hospice care services to patients, families/caregivers
• Ensure all patient’s initial needs are addressed
• Collaborate with the Inter-Disciplinary Team members regarding initial plan of care, patient’s condition, and other essential information

• RN license in the state you work
• Two years of experience as a registered nurse; hospice experience preferred
• Current driver’s license
• Current CPR and BCLS certification
