Summary Of Primary Function/General Purpose Of Position
The Stepdown Registered Nurse (RN) delivers patient care utilizing the nursing process through data collection and assessment, identifying and determining the priority of patient’s problems/needs, and evaluating the process and outcome of nursing care, all in collaboration with the interdisciplinary care team. The RN coordinates, provides, and directs patient teaching activities and coordinates the care provided by health team members.
Essential Job Functions
- Conducts initial and ongoing patient assessment, analyzes assessment data, creates a plan of care, implements treatment, and evaluates treatment effectiveness
- Administers medications in a safe manner consistent with the State of Practice and Bon Secours Mercy Health policies and procedures
- Serves as point of contact for patients' care coordination throughout hospital departments
- Acts as a patient safety advocate by participating in ongoing quality improvement in the department
- Serves as patient advocate in collaboration with spiritual care, palliative care, and ethics
- Serves as a point of care with care coordination and family members or caregivers
- Clinically manages patients with multi-system organ failure and/or multisystem trauma
- Uses the AACN Synergy Model to restore, support, promote, rehabilitate, or palliate to maintain physiologic, psychologic, and psychosocial stability
- Synthesizes and prioritizes information to take immediate evidence-based, patient-focused action
- Anticipates and responds to rapidly changing patient conditions
- Responds to the needs of patients and families coping with illness or injury and advocates for quality of life
- Practices in accordance with the BSMH Nursing Professional Practice Model
Licensing/Certification
- Current RN license in state they are working or covered by compact (required)
- BLS or ACLS (preferred at hire; ACLS required within 6 months of hire)
- CCRN (preferred)
- PCCN (preferred)
- NIH Stroke Certification (preferred)
Education
- Bachelor of Science Nursing (preferred)
Work Experience
- 1 year of RN experience (required); new grads considered
- 6 months RN experience in an acute care hospital setting (preferred)
Training
- EPIC Electronic Health Record (EHR) training (preferred)
- Critical Care Course (preferred)
Skills
- Assigns patient care according to nursing skill, knowledge, and scope of practice
- Orients, facilitates learning, and provides education to nursing staff and others
- Provides evidence-based patient care per standards of practice and hospital policies
- Performs comprehensive assessment of clinical condition and psychosocial needs
- Develops, implements, and prioritizes plan of care
- Applies the nursing process in patient care delivery
- Evaluates treatment outcomes and revises the plan as needed
- Collaborates with patients and families in coping with illness
- Communicates effectively with patients and teams; demonstrates cultural sensitivity
- Maintains professionalism and accountability
- Adheres to infection control policies and uses PPE as required
- Manages chest tubes, hemodynamic monitoring, transvenous/temporary pacing, and conscious sedation
- Knowledge of advanced respiratory modalities
Working Conditions
- Periods of high stress and fluctuating workloads
- Exposure to physical altercations, verbal abuse, and potentially infectious materials
- May require use of restraints and exposure to high noise and bright lights
- Universal precautions and infection control policies apply
Patient Population
- Care for adults 18-64 and geriatrics 65+
- Ability to assess data reflective of patient status and provide age-appropriate care
