Meridian Health Care is looking to hire a full‑time Discharge Planner to join our growing team.

- Act as liaison with other review‑related and discharge‑planning functions and communicate with departments as necessary to ensure coordination of related processes and functions.
- Identify and address any unmet needs of patients.
- Conduct a Discharge Planning Needs Assessment; develop a Discharge Service Plan and update it according to agency policy.
- Monitor and coordinate the delivery of necessary assessments (psychiatric, education, vocational, health, etc.) in a timely manner to ensure that patient’s service needs are adequately identified and a plan to access resources is developed.
- Assist in the coordination and development of the patient’s Behavioral Health Service Plan (Individualized Service Plan, ISP). Review and update the BH Service Plan as required by Meridian Health Care policy, OMHAS standards and as additional needs and goals are identified.
- Provide ongoing one‑to‑one case management services. Services include but are not limited to resource coordination and linkage to financial, housing, social/recreational, mental health/substance abuse, criminal justice, basic living skills and activities of daily living.
- Provide support to family members and significant others, including family education and consultation.
- Assist the client in the development of interpersonal and coping skills, including adapting to home, school, and work environment.

Qualifications- Bachelor’s Degree preferred.
- CDCA, CT, ST, or LSW required.
