Regular Part-Time with potential for full-time expansion As part of the PACE Program team, you will play a crucial role in providing comprehensive care to our patients, many of whom are elderly or have chronic conditions. The ideal candidate will have a strong focus on clinical excellence, patient satisfaction, and a collaborative approach to healthcare. You will work closely with an interdisciplinary team to ensure the best possible outcomes for your patients.

Based on provider staffing, with a minimum 1:3 rotation Daily Duties:
Care planning with the PACE Program interdisciplinary team Serve as the primary care physician for patients in the program Manage and round on PACE participants admitted to long-term care facilities or acute care settings Review medical histories and assessments, and implement plans of care with patients and families Serve as a collaborative physician for advanced practice providers, if applicable Engage in outreach opportunities to expand patient care services This role is ideal for a physician who is passionate about working with geriatric patients and those with complex healthcare needs.

The position also offers the potential for professional growth, with the possibility of expanding into a full-time role as the program continues to grow.

Required Physician License (MD/DO):
