Position: Community Registered Nurse (West) RNA | and Teaching Hospitals NHS Foundation Trust

Are you a Registered Nurse looking for a new challenge, working autonomously within Community Nursing?

We are seeking enthusiastic, caring and motivated Nurses to deliver the best possible care in patient homes and community settings.

Teams Based Locations
  • West Hub – Acomb Health Centre
  • North Hub – Haxby Health Centre
  • Selby/South Hub – Selby Hospital, Tadcaster Health Centre or Sherburn in Elmet
  • East Hub – Tang Hall Clinic
  • South Hambleton and North Ryedale – Easingwold, Pickering and Kirkbymoorside

Occasionally staff will be required to work in a hub that differs from their base. We are community nursing teams comprising motivated professionals, working out of the hospital setting and delivering nursing care in patients’ homes and community settings. We have close working networks with our hospital, social and primary care colleagues to support patients to stay in their homes and avoid hospital admission.

Each day you will work autonomously, delivering patient care to meet patient needs. We are a big team, supportive and friendly, although working independently you will always have the support of the team when needed.

What You'll Do
  • Deliver patient care autonomously in patients’ homes and community settings.
  • Support patients to stay in their homes and avoid unnecessary hospital admission.
  • Work closely with hospital, social care and primary care colleagues to coordinate care plans.
  • Conduct nursing assessments, document treatments and maintain accurate patient records.
  • Provide care in line with NHS standards and the Trust’s policies.
Qualifications & Requirements
  • Registered Nurse with a valid Nursing and Midwifery Council (NMC) PIN.
  • Car driver with access to a private vehicle; mileage will be covered.
  • Ability to work independently while maintaining clear communication with the team.
  • Compassionate, caring, motivated and professional practice.
  • Graduating nurses will receive a 12‑month preceptorship programme, supported by a dedicated preceptor.
