DESCRIPTIONOF WORK
An Aging Care Manager 1 works with older adults from initial contact with individuals, their families and professionals through ongoing care management responsibilities. This includes specialized duties such as protective services, care transitions, and person-center counseling. You will be responsible for maintaining a caseload of consumers and be involved in necessary areas of assistance. You may have the opportunity to regularly coordinate services with physicians, community agencies, and financial institutions.
Managing multiple tasks, paying close attention to detail, completing assignments and meeting deadlines in a timely manner will be essential. With training and team support, the incumbent in this position will be encouraged to move forward in his or her career.

Work Schedule and Additional Information- Full-time employment, 37.5 hours per week
- Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with 60-minute lunch.
- On-Call hours when required.
- Employee may have overnight travel or out of county travel for meetings or training.

- Three years as an Aging Case Aide 2; or
- A bachelor's degree which includes or is supplemented by twelve (12) credits in social sciences, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training.

- You must be able to perform essential job functions.
- You must pass a background investigation.
- A conditional offer of employment will require a drug screening.
- This position falls under the provisions of the Child Protective Services Law.
- Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
