We are seeking a Care Coordinator who thrives at the intersection of clinical knowledge, patient advocacy, and thoughtful follow-up. In this role, you will help patients navigate transitions of care, close gaps in preventive health, and ensure no one feels lost in the system.

• Coordinate follow-up care for hospitalized and emergency room patients, including medication reconciliation and discharge planning
• Support preventive health initiatives by identifying and closing care gaps
• Develop and apply care plans and risk scoring for high-risk patients
• Educate patients one-on-one, including medication setup and chronic care management support
• Address mental health concerns following elevated PHQ screenings with compassion and skill
• Serve as a trusted resource for patients, providers, and care teams
• Assist patients with medication access through patient assistance programs
• Support clinic operations as a triage or refill nurse when needed

• Licensed RN or LPN in the State of Nebraska
• Graduate of an accredited school of nursing
• Current BLS certification
• Care coordination experience preferred
• Strong communication skills and the ability to support patients through complex care needs
