• Admissions, Discharge and Transfers of residents
• Medication requisitions with pharmacy and physicians
• Medication administration
• Follow infection control, emergencies, and other center policies
• Collaborate with resident’s interdisciplinary team to provide the best outcomes
• Identify and report all abnormal changes in residents’ condition to proper personnel per center policy
• Work under direct supervision in accordance with state‑specific Nurse Practice Act, facility policies and procedures, and nursing judgment
• Deliver nursing care to patients/residents requiring long‑term or rehabilitative care
• Collect patient/resident data, make observations, and gloss necessary information related to care
• Implement patient/resident plan of care and evaluate response according to state regulations
• Direct and supervise care given Pod other nursing personnel in selected situations
• Maintain knowledge of documentation requirements
• Maintain knowledge of equipment set‑up, maintenance and use (monitors, infusion devices, drain devices, etc.)
• Maintain confidentiality and patient/resident rights regarding all information

• LPN (Licensed Practical Nurse) licensed by the state
• CPR Certification required
• Two years prior nursing experience preferred
