Job Overview

Physical Therapist (PT) in a Skilled Nursing Facility (SNF) at Flagstar Rehab. Staff PTs in state‑of‑the‑art SNFs throughout NYC and surrounding areas. Work with a multidisciplinary team to evaluate, treat, and educate patients and families to improve physicality and cognition, and help patients achieve better function in daily life.

Direct Employment with our agency OR Independent Contractor. We can assist with setting up a corporation free‑of‑charge.

Hourly Pay: $48 (W‑2) / $61 (1099).

• Provide efficient evaluations and create individualized treatment plans for PTAs to follow.
• Supervise PTAs daily.
• Deliver quality therapy services to SNF residents with TBI, CVA, stroke, Parkinson’s, MS, cancer, orthopedic surgery, and other disabling diagnoses.
• Monitor and document patient progress using innovative EMR software.

• Current NYS PT license and registration.
