Flagstar Rehab works with a diverse, multi‑disciplinary team to perform treatments and provide patient and family education, improving the physicality and cognition of patients and positively impacting their ability to function in daily life.

• Hourly Pay Rate: $31–$34 (W‑2), $35–$38 (1099)

• Collaborate daily with PT Supervisor and follow the established Plan of Care for all patients.
• Provide quality therapy services to individuals in an SNF setting with diagnoses such as TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating conditions.
• Monitor and document patient progress using innovative EMR software.

• Current NYS PTA License and Registration
• EMR Competence
• Meets all State and Federal requirements for US employment in a Nursing Facility
