Primary Care Physician (IM/FM) – Full‑time at Yonkers – Park Care, Sun River Health

Sun River Health is a Federally Qualified, Non‑Profit Health Center serving communities across New York City, Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester. We provide comprehensive primary, preventative and behavioral health services to all who seek care, regardless of insurance status or ability to pay.

• Manage a diverse patient panel with multiple co‑morbidities in a busy Federally Qualified Health Center.
• Diagnose and develop treatment plans for patients.
• Collaborate and supervise advanced provider clinicians.
• Participate in chart review and quality improvement processes.
• Refer and follow‑up patients when needed.
• Provide clinical supervision to students or new providers, ensuring best practices and organizational standards.
• Serve as mentor and guide to developing clinical skills and professional behavior.
• Coordinate educational sessions, workshops or seminars to enhance learning experiences.
• Work closely with academic institutions and other healthcare professionals to support educational activities.

• Doctorate Degree (MD, DO)
• Active/Valid license as an MD or DO in New York State, DEA, Board certification
• Bilingual in English and Spanish (preferred)
• Experience in HIV care and MAT (Medication Assisted Treatment) (preferred)
• Demonstrated ability to work with diverse patient population in community health setting
• Provider must have at least two years of full scope Family Medicine experience
