Overview

In post, you will be supporting the therapy team through managing our service users who have been recently discharged from an acute hospital setting and are undertaking a period of assessment and rehabilitation.

Responsibilities
  • Promote optimum independence through encouraging rehabilitation techniques.
  • Act as a rehab support worker and complete the day‑to‑day rehabilitation patients under the instruction of the lead therapists. Provide feedback on the service users’ progress and help to ensure that they achieve their maximum potential.
  • Undertake a range of delegated clinical and therapy‑led activities to support the service users on the caseload.
  • Provide safe, patient‑centred, effective and evidence‑based clinical activities.
  • Work under direct supervision and be responsible to senior therapists, providing one‑to‑one and group rehabilitation.
  • Work within the Adult Service to deliver a full range of health and social care activities, both acute and long‑term, supported by a robust competency framework for adults within the complex assessment beds.
  • Be part of an integrated multi‑disciplinary team (MDT) that includes nurses, physiotherapists, occupational therapists and other allied healthcare professionals.
  • Operate during service hours 08:00‑18:00 (flexible working can be discussed) with future possibility for weekend cover.
