The Nurse Practitioner serves as a member of a multidisciplinary team providing community-based psychiatric assessment and treatment services to adults and/or children, including those in crisis including in the clinic, home, or community setting. Responsible for providing integrated care, psychiatric care and medication evaluation and management to a range of individuals having Serious Mental Illness (SMI), developmental disability (DD), and/or Substance Use Disorder (SUD) under the supervision of consulting psychiatrist(s) to include 24-hour on-call coverage.

The position also provides assessment, diagnosis, disease management, and prescribing of medications to adults and/or children. The position entails working in a Mobile Unit in multiple locations across the Mount Rogers catchment area. There must be a willingness to travel, make house calls, cover other programs agency-wide especially the Crisis Centers and Crisis Receiving Center. Candidate must have willingness to build a new program and have flexibility to evolve as program needs change.

ESSENTIAL

FUNCTIONS
  • Conducts integrated care, psychiatric treatment, medication assessments, and crisis response to new and existing individuals with SMI, , and/or SUD. Develops individual service plans based upon assessment data, and provides therapeutic interventions as necessary including the provision of ongoing medication management services.
  • Provide care and treatment management of SMI, DD, and/or SUD. Perform on-call rotation to 24/7 crisis stabilization and emergency services treatment team.
  • Pursue and maintain waiver and complete training to provide Medication Assisted Treatment to individuals with opioid use and substance use disorders. Provide Medication Assisted Treatment to individuals eligible for the treatment.
  • Prescribe psychotropic medications when appropriate for disease management. Order and interpret laboratory and diagnostic studies for monitoring of disease progression and pharmacological effectiveness.
  • Complete all documentation required by state law and agency regulations.
  • Provide individualized consultation and health management related to disease management, medication regimen, promotion of wellness, health maintenance, and disease prevention to individuals with SMI, DD, and/or SUD and/or, guardians.
  • Participate in continuing education seminars to maintain current state licensure, prescriptive authority, and professional certification. Keeps mandatory training up-to-date.
  • Conduct agency trainings/presentations related to medical topics/issues upon request.
OTHER DUTIES
  • Provide occasional coverage on a rotational basis for other Mount Rogers' services.
  • Perform such other job related duties as assigned by supervisor.
QUALIFICATIONS
  • Extensive knowledge of: the etiology of SMIs, , and SUD and their treatment by a variety of appropriate therapeutic interventions including psychopharmacology; nursing practice and medical evaluation, physical diseases, and their treatment; community resources and networking techniques; and of psychiatric and medical emergency stabilization.
  • Must have strong clinical skills, to include crisis.
  • Good critical thinking and decision-making skills needed to evaluate and respond to changes in clients' conditions.
  • Dynamic interpersonal skills and therapeutic communication skills needed to work effectively with consumers and other members of the interdisciplinary treatment team.
  • Must be knowledge of the DSM-V, diagnostic screening tools, psychopharmacology, and psychotherapy.

Licensure in Virginia as a Psychiatric Nurse Practitioner preferred or Family Nurse Practitioner with a minimum of two years' experience working with persons who have SMI, DD, and/or SUD preferred.

Strong clinical skills and experience providing treatment to individuals with SMI, , and/or SUD preferred.

Valid Driver's License and a safe driving record required.
