Caring Edge is seeking a Registered Nurse (RN) Case Manager to join our Home Health and Hospice team in Casper, WY
. This unique opportunity allows you to provide patient-centered care in the comfort of your patients’ homes—giving you more independence, deeper relationships, and greater purpose than a traditional clinic or hospital setting.

As an RN Case Manager, you’ll deliver one-on-one care
, ensuring your patients receive not only medical treatment but the compassion, education, and advocacy they deserve. You’ll work closely with providers and the interdisciplinary team to assess, plan, and manage patient care in a truly meaningful way.

• Provide direct care in home-based settings
• Perform wound care, cardiopulmonary assessments, diabetic education, post-surgical and disease management support
• Deliver comfort and education around hospice care, nutrition, medications, and skin integrity
• Collaborate with physicians and interdisciplinary care team members
• Monitor and document progress, educate family/caregivers

• Current active RN license in good standing in the state of Wyoming
• CPR certification (we will provide if needed)
• Valid Driver’s License
