Position Summary
The CNA Team Lead is a certified nursing assistant who provides direct resident care while serving as a shift leader and resource for other CNAs. The Team Lead supports quality resident care, ensures adherence to facility policies and regulatory standards, assists with workflow coordination, and promotes teamwork and communication within the nursing department.
This position does not replace licensed nursing judgment but serves as a peer leader under the direction of licensed nursing staff. This position does not have any supervisory or management authority.
Essential Duties and Responsibilities Resident Care- Provides direct resident care in accordance with the care plan.
- Assists residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, transfers, and feeding.
- Monitors residents for changes in condition and promptly report findings to the licensed nurse.
- Assists with fall prevention and safety interventions.
- Ensures infection control practices are followed.
- Serves as a role model for professional behavior and quality care.
- Assigns and coordinates CNA assignments per direction of licensed nurse.
- Assists with shift organization and prioritization of resident care needs.
- Provides guidance and support to CNAs during the shift.
- Assists with mentoring and orienting new CNAs.
- Helps ensure call lights are answered promptly and care is delivered timely.
- Communicates resident concerns or workflow issues to licensed nursing staff.
- Completes timely and accurate documentation in the medical record.
- Ensures CNA documentation is completed per policy.
- Reports incomplete documentation or concerns to charge nurse.
- Promotes compliance with:
- CMS regulations
- State regulations
- Facility policies and procedures
- Participates in performance improvement activities.
- Assists in monitoring adherence to fall precautions, skin protocols, and behavioral interventions.
- Communicates effectively with:
- Licensed nurses
- Therapy staff
- Social services
- Residents and families
- Assists in shift report when requested.
- Promotes a positive and respectful team environment.
- Current Certified Nursing Assistant (CNA) certification in good standing.
- Minimum of 1 year(s) experience in long-term care preferred.
- Demonstrated leadership abilities.
- Strong communication and organizational skills.
- Ability to work independently and as part of a team.
- Knowledge of infection control and safety practices.
- Ability to lift, transfer, and reposition residents.
- Ability to stand, walk, bend, and reach for extended periods.
- Ability to respond quickly in emergency situations.
- Long-term care facility environment.
- Exposure to infectious diseases and bodily fluids.
- Fast-paced, team-oriented setting.
The CNA Team Lead provides shift guidance and peer leadership but does not perform disciplinary actions unless otherwise designated by administration.
TEAM LEAD/WARD CLERK DUTIES- Ensure B&B charting is done
- Check the bathing charting daily
- See that the breaks are signed for
- Line up the weekly assignments on the board
- Get dietary the weekly appointment information
- Schedule appointments
- Coordinate transportation for appointments
- Register residents for labs
- Take residents to lab appointments
- Vitals daily for nurses
- Ensure stock rooms are stocked
- Scanning documentation
- Attend daily huddle
- Attend weekly IDT
- Ensure CNA sheets are updated
- Daily census sheets are correct and filed
- CNA schedule
- Attend care conferences
- Check brief levels, check rooms for cleanliness and safety concerns
- Help activities when needed
- Help RT when needed
- Get rooms ready when admissions are scheduled
- When residents discharged, medical equipment taken out and sent for cleaning and re-stocking
- Help with all meals in the dining room. If a resident needs to be fed in their rooms, assist them
- Help get weekly weights
