Overview
We are seeking an outpatient orthopedic physical therapist with an interest in pelvic floor dysfunction to join us at M Health Fairview Lakes Clinic in Wyoming.
We treat a wide variety of general and specialty orthopedic and sports medicine conditions. We are colocated in the M Health Fairview Sports and Ortho site on the M Health Fairview Lakes Campus. In collaboration with patients and families, the physical therapist is responsible for providing safe and effective delivery of patient care within scope of practice. This includes examination, evaluation, diagnosis, planning, intervention and establishing outcomes.
FTE: 0.75, authorized for 60 hours per pay period.
Schedule:
ideally 3, 10-hour shifts per week, with flexibility. Full benefits including medical, HSA, dental, vision, 403b, PTO, shift differentials, health & wellbeing resources, and support for continuing education credits.

Responsibilities- The physical therapist partners with the patient and family while performing the Evaluation/Re-evaluation
- Gathers pertinent data
- Performs examination
- Determines treatment diagnosis
- Develops an individualized plan of care through collaboration with the patient and family
- Analyzes data to identify needs and potential risks
- Determines prognosis for recovery
- Establishes individualized goals with patient and/or caregiver
- Individualizes plan of care considering developmental level and cultural differences
- Implements and modifies plan of care based on reassessment and patient response
- Coordinates care and communicates effectively with interdisciplinary team for all aspects of patient care
- Establishes appropriate discharge plan
- Partners with patients and families to provide skilled intervention
- Implements plan of care
- Alters treatment/plan of care to reflect change in patient status and response to treatment
- Assesses patient and/or caregiver learning needs and provides appropriate education
- Physical Therapist in the state of Minnesota or temporary license in the state of Minnesota
- Basic Life Support (American Heart Association or Red Cross)
- Doctorate of Physical Therapy
- 1 year experience

CompensationThe posted pay range is for a 40-hour workweek (1.0 FTE). The actual rate of pay offered within this range may depend on factors such as FTE, skills, education, experience, and market conditions. The organization values pay equity and considers internal equity when making offers. If eligible for a sign-on bonus, the approved bonus program at the time of offer will be honored.
