• Serve as a liaison with referral sources, outside agencies, health professionals, and field staff
• Coordinate incoming calls, providing essential information to patients and referral sources
• Communicate with patients and families about care/service initiation
• Conduct intake screenings, documenting and analyzing necessary information for admission decisions
• Log referrals, prepare reports, and respond to inquiries
• Promote care/services to prospective patients
• Receive verbal orders from physicians to initiate care/services, where permitted by law
• Collect and enter customer information into our management information system

• Graduate of an accredited nursing school
• Current licensure as a Licensed Practical Nurse (LPN) in Michigan
• At least one year of experience as an LPN
• Prior home health care experience is preferred
