In this role, you will:
- Act as a liaison with referral sources, outside agencies, health professionals, and field staff.
- Coordinate incoming calls, providing essential information to patients and referral sources.
- Communicate with patients and families about care/service initiation.
- Conduct intake screenings, documenting and analyzing information for preliminary admission decisions.
- Log referrals, prepare reports, and provide telephonic support as needed.
- Promote our care/services to prospective patients.
- Receive verbal orders from physicians to initiate care/services, where permitted by law.
- Collect and enter customer information into our management system.
To be successful in this role, you should have:
- A degree from an accredited nursing school.
- Current licensure as a Registered Nurse (RN) in Michigan.
- At least one year of experience as an RN.
- Prior home health care experience is preferred.
