In this role, you will:

• Act as a liaison with referral sources, outside agencies, health professionals, and field staff.
• Coordinate incoming calls, providing essential information to patients and referral sources.
• Communicate with patients and families about care/service initiation.
• Conduct intake screenings, documenting and analyzing information for preliminary admission decisions.
• Log referrals, prepare reports, and provide telephonic support as needed.
• Promote our care/services to prospective patients.
• Receive verbal orders from physicians to initiate care/services, where permitted by law.
• Collect and enter customer information into our management system.

To be successful in this role, you should have:

• A degree from an accredited nursing school.
• Current licensure as a Registered Nurse (RN) in Michigan.
• At least one year of experience as an RN.
• Prior home health care experience is preferred.
