A school-based social worker position is available in Wyoming, MI, providing services to middle school students in grades 6 through
8. This contract role offers an opportunity to support students with direct services, consultations, and evaluations in a school environment.

Key Responsibilities:

• Provide direct social work services to a caseload of 35-40 students, with approximately 20 receiving direct support.
• Conduct assessments and evaluations to identify student needs.
• Collaborate with educational staff by contributing to Individualized Education Program (IEP) development, including writing Present Levels of Academic Achievement and Functional Performance (PLAAFP), setting goals, and data collection.
• Maintain Medicaid logging for eligible services.
• Offer consultation to support students' social and emotional well-being.

Qualifications:

• Full Michigan School Social Worker approval.
• Experience working with middle school populations (grades 6-8).
• Proficiency in Medicaid documentation and IEP processes.
• Ability to manage a moderate caseload effectively.
• Strong communication and collaboration skills within a school setting.
