Norfolk Community Health and Care NHS Trust is recruiting for an enthusiastic individual to join the Urgent Community Response South service as a newly created Care Coordinator.

Main duties of the job

Safely and efficiently coordinate urgent patient appointments for Urgent Community Response South. Audit and monitor electronic information to ensure accuracy and quality. Maintain good customer care when communicating with the public and health/social care professionals.

Support basic data management on a daily basis.

About us

The Urgent Community Response South Service operates 7 days per week, including bank holidays between the hours of 8am and 8pm. A requirement to work weekends and bank holidays.

Job responsibilities

Co‑ordinate patient appointments, following set processes, onto ledgers, while considering relevant data including geography, clinical skill mix and staffing levels to ensure patients are seen by the most suitable staff in an urgent timeframe. Scrutinise appointment details against care plans to ensure a correct match and prevent unnecessary visits.

Liaise with health professionals to review referrals and establish the appropriate destination, forwarding them as necessary.

Demonstrate good customer care and communication skills with the public and health/social care professionals.

Recognise the diverse needs and individuality of service users and act with tact and diplomacy.

Demonstrate knowledge of available automated facilities for receiving referrals, finding and checking patient information and storing information in accordance with policies and procedures.

Produce clinical summaries, reports, letters, forms, notices, and minutes of meetings as necessary.

Maintain information stored on the database by updating patient records daily and produce information as requested by team members.

File and store patient notes in accordance with policies and procedures.

Support and contact members of the team to ensure urgent referrals are dealt with within the appropriate response time by managing own time effectively and integrating this with the workings of the team.

Be aware of the Trust and departmental lone worker policy and ensure it is implemented appropriately.

Receive and record messages for members of the teams.

Be an active team member with shared responsibility for general housekeeping by prioritising workload, identifying and exploring own contribution to team working and contributing to the induction process of new members of staff.

Highlight any challenges regarding the service, including any issues relating to quality and pass this to the appropriate person, and propose constructive ideas on improvements to the service.

Monitor and maintain stock levels, requisition and receipting as necessary, demonstrating awareness of budgetary constraints.

Commit to working within a changing environment, responding positively to new demands and changes, and be flexible to the changing needs of the service.

Join the training programme by attending and participating in staff meetings, training sessions and external courses.

Participate in the staff appraisal scheme as an appraisee, maintaining a dynamic personal development programme (PDP).

Demonstrate awareness of clinical governance and risk management and apply it to the work situation.

Person Specification Qualifications
  • GCSE grade C or above, or equivalent in English and Maths
  • NVQ 3 Business Administration or equivalent experience
  • RSA II or equivalent Typing…
