Description:
This position coordinates a variety of nursing care procedures for residents of various ages, which requires professional knowledge and consideration of specific resident conditions and treatment.
The role includes participation in patient and family teaching and provides leadership by working cooperatively with ancillary nursing and other members of the healthcare team.
It is essential to maintain standards for professional nursing practice in the long-term care setting.
Clinical decisions are made independently during the scheduled shift, with complete accountability for all actions taken.
The nurse consults with the appropriate manager or supervisor in challenging situations requiring additional input.
Ability to react calmly and make sound nursing judgments in an emergency environment is crucial.
Accountability for proper delegation and supervision of CNAs, LPNs, and/or Ward Clerks during the shift is required.
The nurse promotes excellence in patient care and implements safe, therapeutic, and efficient care for residents with complex needs.
Setting priorities for care based on acuity and resident preference is necessary.
The nurse anticipates potential problems and adapts to changes by prioritizing emergencies, changes in status, and unusual occurrences.
Initiating, managing, and documenting nursing procedures in accordance with scientific principles and departmental policy is expected.
Knowledge of equipment use and accurate administration of medications is essential.
Carrying out the medical and nursing care plan and coordinating resident care with other healthcare team members is a key responsibility.
The nurse initiates action to reduce, correct, or prevent risks to residents and facilitates healing through individualized, goal-oriented nursing care.
Effective communication with residents and families, explaining tests and procedures, and promoting understanding of health issues are core components.
Serving as a resource for residents facing psychosocial challenges is also important.
Evaluation of resident progress and directing new goal setting based on reassessment is required.
Maintaining the confidentiality of resident information is essential.
Conducting thorough nursing assessments through interviews and observations is part of the role.
This includes documenting findings consistent with other providers and identifying problems and nursing diagnoses.
The nurse must distinguish between normal and abnormal assessments to recognize resident capabilities and limitations.
Engaging in educational activities to enhance decision-making and promote understanding among residents, family, staff, and the community is necessary.
Participation in care conferences and identifying educational needs for staff and residents is expected.
Communicating in a supportive and constructive manner with staff, residents, and other team members is required.
Assisting in the orientation of new staff and providing input on the need for policy changes is part of daily responsibilities.
Developing and contributing to nursing action plans based on resident needs is essential.
Transcribing and noting physician orders accurately is required.
Documentation of reassessments and writing progress notes in the electronic health record is expected.
Active participation in departmental staff meetings and compliance with training and competency checks are necessary.

Familiarity with state reporting guidelines is important.
