Role Overview
The Clinical Care Coordinator is an integral member of the multidisciplinary care team responsibilities include coordinating services for panel patients, especially those who face difficulties navigating the healthcare system; managing specialty referrals; supporting patients in self‑care; identifying relevant community resources; collaborating with and outreaching to outside service providers to ensure seamless transitions of care; and providing patient education while assisting with access to applicable resources.
Required Qualifications- Associate's Degree in a Healthcare Related field, or 2 years of experience in an outpatient setting, or related coursework/training and 1 year of experience in an outpatient setting.
- Bachelor's Degree in a Healthcare Related field
- 2 years of care coordination experience
- Experience in an outpatient setting

$23.81 – $36.44 per hour; additional compensation may be available for shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities.
