Broadwater Medical Centre is seeking an enthusiastic and motivated Clinical Pharmacist to join our General Practice team for a 12 month maternity cover position. Working within your professional boundaries, you will collaborate closely with our multidisciplinary medicines management team and receive ongoing support, development and mentorship from a senior clinician.

Main duties of the job

This is a patient facing role with significant responsibility for chronic disease management, medicines optimisation, and improving prescribing safety across the practice.

About us

We are a practice of approximately 12,500 patients with 4 GP Partners, 3 salaried GPs, 2 paramedic practitioners, a nurse practitioner, 4 practice nurses and 3 health care assistants and a pharmacy team consisting of clinical pharmacist and pharmacy technicians. We also have a full complement of administration and reception staff.

Salary: £22 to £28 per hour (dependent on experience)

Overview

Broadwater Medical Centre is seeking an enthusiastic and motivated Clinical Pharmacist to join our General Practice team for a 12 month maternity cover position. Working within your professional boundaries, you will collaborate closely with our multidisciplinary medicines management team and receive ongoing support, development and mentorship from a senior clinician.

About Our PCN

Broadwater Medical Centre is part of a Primary Care Network alongside St Lawrence Surgery and Victoria Road Surgery in Worthing. Together, we serve a combined population of 41,756 patients. Our demographic is relatively young overall, with a significant cohort of frail patients living both independently and in nursing homes.

Key Responsibilities

Clinical Duties

  • Conduct clinical medication reviews, particularly for patients with complex poly pharmacy.
  • Lead on aspects of chronic disease management within the practice.
  • Provide expert advice on medicines and prescribing to GPs, nurses, and administrative staff.
  • Support the repeat prescribing process, manage acute prescription requests, and undertake medicines reconciliation following transfers of care.
  • Promote safer prescribing systems and contribute to high quality, patient centred care.
  • Support colleagues and patients with medication queries and monitoring.
  • Take responsibility for the care management of patients on hypertensive medication as well as other blood pressure related medication.

Medicines Optimisation & Quality Improvement

  • Provide leadership on one person-centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvements

Contribute to the Quality and Outcomes Framework (QOF), enhanced services, and the local Quality Improvement Scheme (QIS).

  • Identify opportunities to improve prescribing quality, efficiency, and patient outcomes.
  • Use structured medication reviews to support patients to take their medications to get the best from them, reduce waste and promote self-care.

  • Work effectively as part of a multidisciplinary team.
  • Strengthen links between the practice, community pharmacy, and hospital pharmacy services.
  • Support the use of appropriate skill mix to improve access, manage workload, and enhance patient care.
