We’re Hiring:
Care Home Clinician – Newgate Primary Care Network (PCN)
Newgate Primary Care Network is looking for a compassionate and experienced Care Home Clinician to join our growing, supportive team.
📍 North Nottinghamshire | Full-time | Permanent
This role is open to:
- Advanced Nurse Practitioners (ANP)
- Advanced Clinical Practitioners (ACP)
- Paramedic Practitioners
- Other suitably qualified registered clinicians
- (Must hold independent prescribing rights or be willing to work under delegated prescribing.)
You’ll work closely with care homes, GPs, pharmacists, and the wider multidisciplinary team to deliver proactive, personalised care to older adults and community patients with complex needs.
Your work will include:
- Care home MDT meetings and ward rounds
- Community visits and on-site clinics at Newgate Medical Group
- Managing acute and long-term conditions
- Medication reviews and prescribing
- Palliative and end-of-life care
- Supporting care home staff with training, prevention, and self‑care approaches

About UsNewgate Medical Group is a single‑practice PCN with over 30,000 patients and responsibility for 6 care homes. We are a training practice with a large, diverse and friendly team including:
- GP Partners, ACPs, ANP, pharmacists & pharmacy technicians
- Care coordinators leading vaccinations, home visits, care home reviews, safeguarding and long‑term condition management
- Strong admin, IT, HR and finance support teams
- Experienced nursing, HCA, phlebotomy and diabetes specialist staff
We’re based near Clumber Park with great transport links to Sheffield, Doncaster, Chesterfield, Newark, Mansfield – and London is under 1.5 hours by train.
What We’re Looking ForEssential:
- Registered with NMC / HCPC / GPhC
- Independent Prescriber (or working under delegated prescribing)
- Experience in primary or community care with complex patients
- Full driving licence and own vehicle
Desirable:
- Care home or older adult care experience
- MDT working and proactive long‑term condition management
- Supportive, inclusive working environment
