Onboarding Process

Please be advised that candidates must successfully complete a background check and pre‑employment health screening which includes a drug screen.

Position Summary

Under general supervision, the person in this role provides navigation assistance to patients receiving care in assigned areas. The individual identifies patients with specific diagnoses and connects those patients to coordinated care management. They may provide referrals, appointment scheduling, and/or lifestyle education to patients as specified in program procedures. The individual educates patients on care management options in accordance with policies and procedures.

They maximize the number of individuals receiving access to primary care. They may elicit patient consent to enroll in primary care‑based ACO and Case Management programs. The person communicates with patients about ACO and Case Management programs and models while offering referrals as needed and as specified. They maintain patient confidentiality and adhere to HIPAA laws, provide excellent customer service, collaborate, and cooperate with care team members to ensure consistency and quality of care and promote productive employee relations among staff members.

Qualifications

Education
  • Required:

    High school diploma or equivalent
Experience
  • Required:

    1 year related experience in an outpatient clinic or hospital setting, including 6 to 12 months data entry experience
  • Preferred: 1 to 2 years of work experience in insurance, medical records, billing, or related area
