Overview
Adults Social Worker (CAP) – We are seeking a dedicated and compassionate Adults Social Worker (CAP) to join our dynamic team and make a meaningful impact in the lives of adults in our community. This role is vital in ensuring that vulnerable adults receive the support and services they need to lead fulfilling lives. As an Adults Social Worker, you will conduct assessments, develop personalized care plans, and advocate for the needs of your clients.
You will work closely with other professionals and stakeholders to provide integrated support and navigate complex systems, ensuring clients have access to resources such as healthcare, housing, and social services. This role offers an opportunity to engage with individuals from diverse backgrounds and create tailored solutions that empower them to achieve their goals and well-being.
- Conduct comprehensive assessments of adults' needs and circumstances to inform care planning.
- Develop and implement person-centered care plans in collaboration with clients and their families.
- Advocate for clients' rights and access to services, ensuring their voice is heard in all processes.
- Collaborate with healthcare providers, community organizations, and government agencies to coordinate services.
- Monitor and review care plans and interventions to ensure effectiveness and suitability.
- Provide emotional support and practical assistance to clients facing challenges in their daily lives.
- Maintain accurate and timely documentation of case notes, assessments, and care plans as per regulatory standards.
- Bachelor's degree in Social Work (BSW) or related field;
Master's degree preferred. - Valid social work license or certification as required by state regulations.
- Proven experience working with adults in a social work or community service environment.
- Strong knowledge of adult safeguarding policies, mental health, and social care regulations.
- Excellent communication and interpersonal skills to build rapport with clients and stakeholders.
- Ability to work independently and as part of a multidisciplinary team in a fast-paced environment.
- Strong organizational skills with the capability to manage multiple cases and priorities effectively.
Hours per week: 37.00
Start Time: 09:00
End Time: 17:30
Pay per hour: £21.31
