Seeking a full-time Nurse Practitioner or Physician Assistant to provide primary care in Assisted Living Facilities around Hyannis, MA.
- Perform primary care visits, medication reconciliations, and annual wellness exams five weekdays.
- Examine patients, review medical records, order and interpret diagnostic tests, diagnose conditions, and prescribe treatments and medications.
- Educate patients and families on health, disease prevention, and management.
- Maintain accurate records, promote health practices, and oversee overall patient wellbeing.

Work involves in-person visits with an emphasis on delivering care within assisted living and memory care settings.
