Hospice Registered Nurse Case Manager
- Completing an initial assessment of patient and family to determine hospice care needs
- Regularly re-evaluating the patients’ nursing needs
- Use assessment data to determine care plan needs
- Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide care that is continuous and well-coordinated
- Counsel the patient and family in meeting end of life needs
- Initiate appropriate preventative and palliative nursing procedures
- Communicate with the physician regarding patient’s needs and report any changes in the patient’s condition
- Supervise the Hospice Aides
Education: Associate's Degree in Nursing
- Must have a State Certified RN license
- 2 years' experience as a professional nurse, hospice experience preferred
- Must have a valid Driver's license and reliable transportation
- Must be willing to travel
- Strong verbal and written communication skills.
