We are currently hiring a Community Health Worker – Behavioral Health based in Worcester. As part of the care management team, the Community Health Worker (CHW) works as a part of the health care team to provide comprehensive, coordinated CHW interventions in accordance with the Massachusetts Board of Certification of Community Health Workers core competencies as part of a patient centered health care home for Health Center patients.

Essential

Functions
  • Provides health system education and advocacy to help patients navigate care and access needed resources.
  • Supports patients facing mental health, emotional, or substance-use challenges with culturally appropriate connections to services.
  • Conducts SDOH assessments and creates intervention plans based on identified needs.
  • Assists patients with housing, transportation, education, employment, child care, food security, and utilities.
  • Follows up with referred patients to address social barriers affecting engagement in care and treatment adherence.
  • Participates in population-based projects and meets grant-related requirements.
  • Performs other duties as assigned.
Required Qualifications
  • Minimum 1 year of work or volunteer experience in human services or healthcare with demonstrated knowledge of or ability to work with the targeted community.
  • Basic knowledge of Microsoft Office and ability to learn the health center’s Electronic Medical Record and other applications.
  • Excellent written and oral communication skills.
  • Successful completion of the CHW core competency training within 2 years of employment.
