St Johns House Medical Centre, Worcester is looking for a committed salaried GP to join our practice team to help shape our future growth. We have strong connections to our PCN, with a great multi-generational patient population .
The Practice
List size of 13,800
An innovative and supportive team, comprising of two partners, six GPs, three trainee ACPs, two practice pharmacists, three nurses, three HCAs, a practice manager, an operations manager, reception managers, receptionists, administrators and secretaries.
The Practice is consistently benchmarked as high performing in ICB and PCN contracts across a range of indicators, including referrals and prescribing.
Bi-weekly MDT with social and community care.
Training practice.
Main duties of the jobThe Role
We are offering 6 sessions working on Monday, Thursday & Friday.
Interests and personal development strongly encouraged.
Support for being/becoming a trainer for interested candidates.
Mentorship for portfolio roles, including future leadership roles in the wider community/health care

Job responsibilitiesClinical Responsibilities:
Thepost-holder is expected to take due note of and act in accordance with the standards of clinical governance that are agreed within the practice. Any concerns should be brought to the attention of the partners as soon as practicable.
In accordance with the practice timetable, the post-holder will make him/herself available to undertake a variety of duties including: surgery consultations,telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions, taking action on electronic prescription requests, and dealing with queries,paperwork and correspondence, (including electronic correspondence), dealing with patient test results and medical reports, referral letters, paperwork andother correspondence in a timely fashion
Make professional, autonomous decisions in relation to presenting problems, whet herself-referred or referred from other health care workers
Assessthe health care needs of patients with undifferentiated and undiagnosedproblems
Screen patients for disease risk factors and early signs of illness
In consultation with patients and in line with current practice disease management protocols, develop care plans for health
Provide counselling and health education
Admitor discharge patients to and from the caseload and refer to other care providers as appropriate
Record clear and contemporaneous consultation notes to agreed standards, primarily byuse of the computer system and complete the clinical system clinical templates as appropriate
Collect and use data for audit purposes
Compile and issue computer-generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible)
Prescribe in accordance with the practice standards and locally agreed or national guidelines whenever this is clinically appropriate
Ingeneral the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
