Exemption Status: Non-Exempt

Hiring Range: $17.53 - $26.68

Final offer may vary within this range based on a candidate's experience, skills, qualifications, and internal equity considerations.

Major Responsibilities
  • Provides constant observation of high‑risk patients as assigned by the Emergency Department Resource Nurse.
  • Assists patients to get undressed, removes items that could injure the patient or staff, secures belongings.
  • Monitors patients and conducts safety checks every 15 minutes for an assigned group of patients.
  • Escorts and supervises patients in the bathroom depending on level of care, assists patients with meals and comforts, de‑escalates patients by using sensory techniques and distraction techniques as needed.
  • Attempts to prevent unsafe patient movement and actions.
  • Notifies the nurse of changes in patient activity, mood, behavior, requests for medication and calls for assistance when necessary.
  • Reminds patients about safety and fall prevention, redirects patient attention from equipment, and prevents patients from dislodging equipment.
  • Uses de‑escalation techniques and, if unsuccessful, participates in restraints under RN or MD direction in accordance with training and education.
  • Obtains a report on each assigned patient from the nurse at the beginning of each shift.
  • Reports any potentially unsafe behaviors or patients who appear to be escalating to the assigned nurse throughout the shift.
  • Performs environmental safety checks and removes potentially harmful items at the beginning of each shift and between discharges.
  • Remedies hazardous situations such as improperly open doors and windows.
  • Follows the Policy & Procedure(s) for Constant Observation.
  • Accompanies patients for therapeutic procedures unless otherwise ordered by the nurse or physician.
  • Documents patient behavior every fifteen minutes on the Constant Observation documentation record.
  • Provides handoff to the on‑coming ED Observation Assistant.
  • Supports patient care and department operations with a variety of duties.
  • Engages patients in activities as appropriate.
  • Maintains a neat and orderly work area, stocks carts and supplies, transports patients or equipment.
  • Answers telephones, takes messages, and completes light clerical duties such as filing as assigned.
  • Follows infection control, fire safety, and other departmental policies and procedures.

  • Complies with established departmental policies, procedures and objectives.
  • Attends meetings, conferences, seminars as required or directed.
  • Uses Quality Improvement in daily operations.
  • Complies with all health and safety regulations and requirements.
  • Respects diverse views, demonstrates Standards of Respect, and contributes to a professional, tolerant environment.
  • Maintains reliable attendance.
  • Performs other similar and related duties as required or directed.
  • All responsibilities are essential job functions.
Position Qualifications License/Certification/Education
  • High School Graduate or equivalent.
  • Must complete de‑escalation training and a Basic Life Support or Heartsaver course and receive appropriate certification.
