Hospice Social Worker (MSW)

Acclaim Home Health is seeking a compassionate and dedicated Hospice Social Worker (MSW) to join our interdisciplinary hospice care team in Worcester, MA. The Hospice Social Worker plays a vital role in supporting patients and families through end-of-life care by providing emotional support, counseling, care coordination, and access to community resources.

• Provide psychosocial assessments, counseling, and emotional support to hospice patients and their families
• Assist patients and caregivers with coping strategies related to terminal illness, grief, and end-of-life planning
• Coordinate with the interdisciplinary hospice team (nursing, chaplaincy, aides, physicians) to develop and implement patient-centered care plans
• Connect patients and families with appropriate community resources, financial assistance, and support services
• Participate in care conferences, team meetings, and discharge planning as needed
• Provide crisis intervention and bereavement support
• Maintain accurate and timely documentation in accordance with hospice regulations and company policies
• Ensure compliance with state, federal, and hospice regulatory requirements

• Master’s Degree in Social Work (MSW) – REQUIRED
• Current Massachusetts Social Worker license (or eligibility to obtain)
• Hospice or healthcare social work experience preferred
• Strong communication, counseling, and organizational skills
• Ability to work independently and as part of a collaborative care team
• Compassionate, patient-focused approach to care
• Reliable transportation and ability to travel locally as needed
