Position Summary

The Chronic Disease Registered Nurse provides direct and indirect nursing care in a family practice setting. The RN will provide assessments, disease management, education, training and other clinically based activities to coordinate care among providers, patients and the community. The RN will participate in all aspects of care to patients including triage, health maintenance teaching, and administration of vaccines/medication. The RN will also collaborate and communicate with other nursing professionals, medical assistants, medical providers and community health workers, to ensure high quality patient care.

Program Specific

Essential Duties and Responsibilities:

  • Triages patients in person and by telephone
  • Provides clinical care under the supervision of a provider
  • Assess patient status regularly.
  • Administers medications and carries out procedures as ordered by providers.
  • Completes required documentation in the electronic medical record.
  • Follows-up on abnormal lab results, and works with providers caring for patients with abnormal labs.
  • Assists Health Center providers in providing comprehensive health care that is consistent with the philosophy, mission and goals of the Health Center.
  • Provides health education and information to patients through individual and group methods
  • Serves as a liaison for patients, facilitating access to other Health Center services and resources.
  • Participates in and promote specialized programs which further the well-being of Health Center patients
Supervisory Responsibility:

Precept medical assistant(s) and/or community health worker(s) by assisting and providing direction with procedures and activities that facilitate patient centered care.

Qualifications and

Education Requirements:

One of the following combinations of education and employment experience must be met to be considered for the position:

Education and Experience:
  • Graduate of accredited school of nursing required. AS or BS in nursing preferred
  • Two years full time professional experience or part time equivalent
  • Must be licensed as a RN by the Massachusetts State Board of Registration. BLS required
