Fallon Health is seeking a Health Aide/CNA to provide personal care and assistance with daily living activities to participants in their homes and at the PACE center.
Several shifts are available, and we offer full-time benefits, health insurance, paid time off, and a 401K savings plan.
Responsibilities include:
- Assisting with daily living activities such as bathing, dressing, and toileting
- Contributing to the development of care plans for participants
- Collaborating with the interdisciplinary team to ensure continuity of care
- Maintaining a clean and safe working environment
- Providing accurate and timely documentation
Qualifications:
- Completion of an approved Home Health Aide or Certified Nursing Assistant Training Program is desirable
- Certification as a Home Health Aide or Nursing Assistant is desired
- One year of experience working with a frail and/or older adult population is preferred
