Performs a variety of duties involving patient assessment, psychotherapy and coordination of services. Collaborates as a member of inter‑disciplinary treatment team for the development and delivery of treatment and discharge plans for psychiatric patients. This position will provide and present summary reports and recommendations to Vice Chairs and Vice Presidents.
Major Responsibilities
Compiles data and records according to specific requirements and prepares summary reports for Vice Chairs, Vice Presidents, and others.Present findings from projects and initiatives to Vice Chairs, Vice Presidents, specific committees and others.Assist Administration and department leadership to evaluate impact of interventions and develop ongoing programs.Initiate extramural funding efforts through grant writing in order to defray program expenditures. To be reviewed annually by department management.Conduct intake interviews with patients and families to develop psychosocial evaluations.Provide clinical service to patients and their families through individual, family or group work, such as psychosocial assessment, individual education and treatment, crises intervention, treatment planning and counseling relative to medical conditions.Identify clinical resources and make referrals when necessary. Work with outpatient providers to coordinate treatment planning.Collaborate with physicians, care coordinators, discharge planners, and other health care professionals to identify barriers to discharge or treatment and help resolve these issues with patients, families or external sources.Advocate for patients and families as they negotiate the health care system so that effective care, treatment and planning may occur.Document all information obtained in evaluation, assessment, consultation, referral and disposition to comply with regulatory requirements, as well as ensuring best possible patient outcome.Provide social service consultation, in‑service, and education to medical, psychiatric, and social service providers within scope of practice.Act as a liaison with other social workers or social service agencies as needed.Facilitate referral for psychiatric services and access to community social service agencies to assist patient to resume life in the community.Utilize the language of the current DSM‑IV‑TR to establish comprehensive diagnostic evaluations and subsequent planning and treatment.Position Qualifications
License/Certification/
Education:
Required:
MA in behavioral health, social work, or counseling.Preferred:
Master's Degree in social work.Current Massachusetts Licensed Independent Clinical Social Worker (LICSW) licensure.Candidates with independent licensure as Licensed Mental Health Counselor (LMHC) and/or Licensed Alcohol and Drug Counselor (LADC) with greater than three years of experience with demonstrated proficiency in the areas of responsibility may be considered.Experience/
