If you are passionate about supporting children, youth, and families, and have strong skills in care coordination and collaboration, we invite you to join our Intensive Care Coordination (ICC)/CSA team as a Care Coordinator II. In this role you will facilitate the Wraparound process for youth ages 0–21 with behavioral/emotional needs, helping them and their families access critical supports and services.
Key Responsibilities- Provide high‑quality Wraparound services for children/youth and families.
- Conduct comprehensive assessments, including CANS, CRAFFT (12+), diagnostic assessments, and Serious Emotional Disturbance determinations.
- Convene and facilitate Care Planning Team (CPT) meetings to develop and update Individualized Care Plans (ICP).
- Maintain consistent communication with youth, families, and care teams to identify strengths, needs, and effective strategies.
- Support youth and family participation in goal‑setting and implementation.
- Identify and connect families with medically necessary services.
- Collaborate with state agencies, schools, healthcare providers, and community resources.
- Develop transition plans when goals are met.
- Obtain insurance authorizations as required by Managed Care Entities.
- Maintain service productivity and caseload expectations.
- Ensure timely documentation and completion of Risk Management/Safety Plans.
- Master’s Degree in a Human Services‑related field (license‑eligible).
- CANS‑MA certification (required).
- Knowledge of mental illness/SED in children/adolescents and treatment techniques.
- Familiarity with internet, word processing, and email systems.
- Experience navigating child/family‑serving systems (preferred).
- Knowledge of community resources (preferred).
- Experience facilitating meetings or groups (preferred).
- Demonstrated cultural competence and ability to work with diverse populations.
- Strong organizational, communication, and facilitation skills.

Salary: $69,243.00
