Intensive Care Coordinator (ICC):
Intensive Care Coordination is based on the well-known principles of Wraparound Care that respects and incorporates the needs and choices of the family with a focus on family culture, their strengths and their voice. The Care Plan Team is chosen by the family and the team helps inform the family on the resources, supports and interventions that can support the family’s goals.
Coordination of Care has shown to have better outcomes in improving the lives of children and families and is a key component of our Children’s Behavioral Health Initiative (CBHI) System. Care Coordinators work as a team with a Family Partner, an individual with lived experience parenting a child with emotional/behavioral needs.
Qualifications- Master’s Degree in a mental health field (e.g., counseling, family therapy, social work, psychology, etc.) from an accredited college or university.
- Bachelor’s Degree in a human services field from an accredited college or university and one year of relevant experience working with families or youth.
