The Health Homes Coordinator supervises daily nursing operations, manages case loads, ensures compliance with state and federal regulations, supervises staff, and supports quality patient care in an outpatient opioid treatment program (OTP).
Key Responsibilities- Supervise case managers
- Manage case loads and ensure high‑quality, coordinated patient care
- Lead quality assurance and compliance activities
- Oversee medical record documentation
- Collaborate with internal teams and external community partners
- Engage in community outreach and resource coordination
Calculated salary range: $77,000.00 – $87,000.00 per year.
Role LocationRhode Island
Qualifications- Active RN license in Rhode Island
- Master’s degree in a relevant field (preferred)
- Experience in case management, managing caseloads, and supervisory leadership
- Experience in community outreach and connecting patients to local resources
- 2+ years of nursing experience (behavioral health preferred)
- 1+ year of supervisory or case management experience
- No history of substance misuse within the past 3 years
- Medication‑assisted treatment (methadone, buprenorphine, naltrexone)
- Behavioral health or addiction recovery settings (outpatient, residential, correctional)
- Knowledge of trauma‑informed care, harm reduction, and regulatory compliance (DEA, HIPAA, Joint Commission)
- Experience with clinical documentation and billing processes
