Job Description

Join a reputable organization specializing in long-term care, skilled nursing, acute care, and hospice services as an Intake Nurse. This office-based role is ideal for nurses looking to transition from hands-on patient care to a more administrative setting. You will play a crucial role in coordinating referrals, verifying and entering physicians' orders, and identifying the required level of care for patients.

Responsibilities

* Coordinate patient referrals and verify physicians' orders.

* Identify the appropriate level of care required by patients.

* Review documentation to determine facility placement.

* Coordinate orders with pharmacists.

* Collaborate closely with referring hospitals, physicians, skilled nursing facilities, patients, and families.

Essential Skills

* Licensed Practical Nurse (LPN) or Registered Nurse (RN) licensed in the State of Utah.

* At least 1 year of experience, preferably in a skilled nursing facility.

* BLS (Basic Life Support) certification.

* Experience in referral coordination, skilled nursing, and long-term care.

Additional

Skills & Qualifications

* Experience with Matrix Care or Point of Care Electronic Medical Records (EMR).

Work Environment

Work in a professional office setting with a business casual dress code. The team consists of 32 office staff, with 9 members on the immediate team. The schedule is a rotating Monday-Friday shift between 8:00 AM and 7:00 PM, with a weekend shift and on-call duty every 6-8 weeks. The role is based in an office building, separate from the care facility itself.

This is a Contract to Hire position based out of Woods Cross, UT.

Pay and Benefits

The pay range for this position is $32.00 - $32.00/hr.
