Our multidisciplinary teams deliver personalized care in patients’ homes.

• Provide in‑home, para‑professional services under RN direction.
• Assist with personal care activities (baths, showers, skin & hair care, oral hygiene).
• Notify RN of any changes in patient condition promptly and document appropriately.
• Record all required information on patient chart accurately and completely.
• Assist patient with transfer, ambulation, and perform range‑of‑motion exercises ensuring safe body mechanics.
• Maintain general housekeeping of patient area and change patient linens.
• Assist client with self‑administration of oral medication and remind patient when to take medication.
• Prepare simple meals according to dietary instructions and assist patient with eating if necessary.
• Assist with Foley catheter and colostomy care, ensuring proper hygiene and skin cleanliness around stoma.
• Perform other duties and projects as assigned with demonstrated flexibility.
• Adhere to HMH organizational competencies and standards of behavior.
• Lift a minimum of 50 lbs, push and pull a minimum of 50 lbs, and stand for a minimum of 8 hrs a day.

• Good customer service skills.
• Home Care experience preferred.

• Home Health Aide (HHA) certification.
• Valid driver’s license from a U.S. state.

Minimum rate of $19.33 per hour.
