The
** Pediatric Specialty Clinic - Woodbury
** is a free-standing clinic that offers a wide range of services to pediatric patients seeking care from any of our several sub-specialties.  Some of the pediatric sub-specialties we see include;
Neurology, Cardiology, Endocrine, Diabetes, Gastroenterology, Rheumatology, Weight Management, Sleep medicine, Pulmonology, Surgery, NICU Follow-Up, Nephrology, Dermatology and Urology. Many of our clinics are multi-disciplinary, which means we offer multiple specialty services within one clinic appointment. We are a supportive, cohesive team that is passionate about pediatrics.
** What you will do as an
** RN Care Coordinator**:
*** Coordinates care across the health care continuum within designated sub-specialty population(s)
* Serves as a single point of contact by facilitating and coordinating transitions in care within and between care settings, providers, and services
* Completes pre-visit planning and post-visit wrap-up and determines vital follow-up appointments and/or treatments to optimize patient visits
* Facilitates proactive disease management and preventative health care using evidence-based practice
* Rooms patients for the surgeon(s) as needed, complete clinic readiness including records review and provides administrative support as needed.

* What you will need:

*** Current Minnesota RN license
* 3+ years of Clinical Nursing experience
* BLS certification
* Demonstrated experience in patient advocacy and education, coaching, and proactive health care management along with EMR/digital literacy
